Applied online and got a phone call the next day to set up an interview. The position was "RN supervisor", which is a staff RN position with a fancy name. I interviewed with the DON, who looked exhausted. Turns out she had interviewed 3 people before me and had 6 more interviews after me! I know the person before me got a job, too, because I saw her walking out with drug screen paperwork. Needless to say, turnover at this place must be astronomical. There were so many shifts open, I was able to pick any shift I wanted. After talking to me for less than half an hour, the DON walked me over to the HR office and told the woman working there to schedule me for a drug test and a PPD. At no time did she ever say, "I'd like to extend an offer to you". It was just assumed somehow that I was being offered the job. I have never been to an interview where the offer was immediate, and it really made me wonder how desperate this place is. No one ever asked me to show my RN license or my CPR card.
Offered pay was $30.00 an hour with NO benefits. Part-timers get no benefits at all.
Things that made me uneasy:
The patient load at this sub-acute rehab facility is 20 patients per nurse. I know what a ridiculous and unsafe patient load that is. These are people who have just left the hospital and can have anything from a hip replacement to a brain injury. It is NOT the same as a "nursing home", where the patients have been there long-term and are usually more medically stable.
Medications must be passed within a 2-hour window (2 hours EXACTLY, I was told), starting exactly 1 hour before the medications are scheduled. Passing meds to 20 patients in 2 hours leaves 6 minutes per patient. And many will be getting cardiac and blood pressure meds that require a BP and pulse be taken first. All it takes is a couple of patients who are very slow at swallowing, or a balky BP cuff, etc. and your 2-hour med pass turns into a nightmare race to the finish.
They are understaffed with CNAs, who usually have to handle 10 patients per shift. Also, the CNAs at this facility don't do vital signs and they don't do blood sugars, so the nurses must do them. (Their website advertises that "experience is not required" for CNAs. Great, who wants to work with a crew of inexperienced CNAs all day, every day?) It was suggested to me that I could come in early to take vital signs, but clocking in early is not permitted. Working before and after clocking-in/out is a liability for both the nurse and the facility.
Much of the charting is still done on paper. Think of charting 20 patients on paper and how long that takes. (Remember, shifts are 8 hours.)
RNs do both medications and treatments for 20 patients. So after you do your med pass, you get to start wound care and any other treatments. At many facilities, one nurse does treatments and another nurse does medications. But at HCR, nurses do it all. For those of you not familiar with wound care, it is extremely time-consuming. And remember, it is likely you'll have to help some of your patients dress and bathe and use the bathroom/commode/bedpan, because the overworked, inexperienced CNAs will be running around like crazy, trying to get everything done.
I was told that corporate policy is NO bed alarms, NO wheelchair alarms, and NO RAILS on the beds. Not even 2 rails! I was told point-blank that "we have plenty of of falls here". Yes, and who is going to get the blame when a demented patient falls out of her wheelchair? The nurse, right.
One of the staff RNs had to place my PPD (tuberculosis test). As if they don't have enough to do, without placing PPDs for the masses of job applicants! I'm assuming the company is just too cheap to send you to a physician or to have an employee health department, even in a facility with over 100 beds. I asked the nurse if she liked working there, and she said, "um, yeah". Very flat and unenthusiastic. And oh, they clip your health history to a bulletin board, for all the world to see. Did someone say HIPAA?
Nurses were loudly discussing the 16-hour shifts they were working, while other nurses were being urged to work multiple shifts. It sounded to me like it is routine for this to happen. Not that I could realistically expect to finish my normal shift in 8 hours on most days.
There is no RN supervisor ("supervisor" for real). It's just the nurses and the DON (didn't have an ADON there) when she is there. The DON was unconcerned about my relative lack of experience, despite my raising concerns to her. I'm sure my fellow nurses would have loved to help me with unfamiliar nursing procedures while also taking care of their 20 patients. NOT!
The company is terrified of unions, and has a ludicrous "Pro-Employee" statement on their corporate website. Any company that is that afraid of unions must have something to hide.
Overall, I think my main qualification for the job was being an RN and having a pulse.

I felt the staffing level is simply unsafe at this facility. And the lack of clinical support (no RN supervisor, no educator, etc.) is downright dangerous for a new-ish nurse. I kept thinking about trying to pass multiple meds to 20 patients in 2 hours. Patients like this can easily have 6-10 or more meds each. Bottom line: I felt like my license would be in jeopardy and I would be unable to provide quality care to each patient. And I felt that if I made a mistake, or my patient fell, I would be thrown under the bus. Especially after reading some ManorCare horror stories online. Thank you, but no thank you.

I was interviewed by 5 people for 3 in-person interviews and 1 phone interview. I took a personality test. The process was long and I received 2 for rejection letters which were not intended for me as the interview process continued after I received them. There was a lot of confusion during this process. One interviewer said I was the only person that was being interviewed and another told me there were others being interviewed.
