Interviewed Oct 2011 in Northbrook, IL (took 1 week)
You apply online through their website which entails sending in your resume and references as well as taking two tests: The first is a basic skills test that includes vocabulary, reading, and math problems; the second is a personality test.
Once submitted, if they like what they see and are looking to hire, they will contact you immediately and schedule an interview. The interview is typically one-on-one with the Clinic Manager, but if you're lucky, the Clinic Owner could be present and he'll sit in for the interview (don't worry, he's nice and easy-going!).
If the interview goes well they will either schedule a second interview, contact you shortly after to offer you the position, or hire you on the spot. The interviews are very laid-back, so don't be nervous. It's all about showing off your personality and capability of selling memberships to people.
My suggestion, which was very successful for me, would be to look over their website and become familiar with the membership and benefits of massage therapy. You are applying for a sales position, not just a receptionist, so you have to be able to sell and be knowledgable of what you are selling. Also, if you are reading this prior to your interview, you already have an advantage and will be a lot more prepared than most :) Show confidence!
The entire interview is basically asking standard questions to get to know you. Be prepared to "sell" a membership to them because they will spring this on you! It doesn't need to be perfect, just show that you can. Once you wow them with your answer and personality, you've got the job ^_~

Interviewed Aug 2010 in Scottsdale, AZ (took 2 days)
Walked in to fill out an app - interviewed me on the spot, one on one interview. Asked me to fill out personality test. Asked questions, and then asked me to come back the next day to meet with senior management and memorize a script overnight.
I barely remembered the script but they didn't seem to care...

Interviewed Aug 2009 (took a day)
Massage Envys are franchises. I first interviewed with one franchise in June. The job was advertised on craigslist and I was contacted via email to come in for an interview. While waiting for the interview to begin, I noticed the place was dead. I was asked to fill out an app and while doing so, 4 others arrived. It was a group interview, which was conducted by the male franchise owner. The manager sat wordlessly beside him the whole time. We, the interviewees, had to tell a little about ourselves and then we were asked questions like: what motivates you, have you ever had a massage, how do you feel when people lie to you, what have you learned about the person sitting next to you, etc. Then we had to sell something. Our choice was a pen or stapler. I received an email the next day, inviting me to come back in to meet with some coordinator or asst manager or some title like that. I was there 5 minutes. What a waste of time. She just wanted to hand me their "script". Heck, she could have EMAILED it to me, rather then have me waste a 30 minute drive. The girl told me to memorize it but to personalize it and call her in a couple days. I asked about pay and was told $8 base but with commission, I should easily make 12-14 an hour. Again, I noticed the place was dead. I went home and memorized the script but changed things around to "personalize" it as I had been instructed to do. I called back a couple days later and was asked to come in and do my presentation. So, back to Massage Envy I go, this time I deal with the manager. I recite the script, adding my own twist to it. She was annoyed, telling me no no no, you have to recite it WORD FOR WORD. Then she asked me to sit behind the front counter, to "get a feel for things". There were 2 employees behind the counter. I sat there for about 20 minutes. Surprise surprise, the place was dead. One of the employees admitted to me it was almost always dead. I asked how she made any money and she just rolled her eyes. After about 15 minutes,I made an excuse to leave and emailed them I wasn't interested. I worked a few weeks for a temp agency and then one day, while browsing Craigslist, I noticed another franchise owner advertising for sales associates, with the pay listed as $11-$15 an hour. I thought, well why not try them, looks like they are paying decent wages. So I applied and received an email to come in for an interview. I went and was asked to feel out an app. Slowly, others began to arrive for their interviews. I thought OH CRAP, its going to be this group interview mess again. But this time, instead of just 4 others, about 15 people were trickling in. The girl behind the counter kept walking around the corner where the "director" was and coming back smirking. It was really annoying. The director kept everyone waiting for over an hour. Finally, she emerged and we all walked back this tiny room that was big enough to hold about 5 people max. But, its about 18 of us total. The director informed us that her new manager is going to handle the interview. He was incredibly young and rather unprofessional. The interview took forever to get off the ground, as the director and manager kept joking with each other about inside jokes and things going on in the office. People were rolling their eyes. It took a good 20 minutes before the interview even started, with everyone looking blank as the director and manager kept up their back and forth banter. The interview was the same mess as before. Everyone had to tell something about themselves. The same stupid questions were asked as at the previous group interview I attended. The manager doing the interviewing didn't call on any particular person to answer the questions, he just threw them out there and so you had multiple people talking at the same time. It was the dumbest thing I have ever seen in my life. I seriously thought about walking out. One woman finally did and then another. Someone asked about pay and I was not surprised to hear $8 base plus commission. So much for the 11-15 mentioned in the ad. The director continued making jokes and interrupting people every few seconds. Then she suddenly got serious and started talking about the business being a hive and the workers as the bees. Huh? This entire fiasco took 3 hours, and yeah, we had to sell a pen or stapler. I was furious and people were complaining as we exited the building. Everyone was disgusted as not only had they expected a 1:1 interview, they also did not expect in their wildest dreams they would be crammed in a tiny room and have to sit through a 3 hour group interview. I wasn't offered a job and you know what? That was more then fine with me.

Interviewed Feb 2009 in Raleigh, NC (took 1 week)
I was looking for jobs on Craigslist and came across a listing for Massage Envy. The listing basically showed what the job entails and listed that you would be paid anywhere from $9-$13 (this is misleading in itself but I will get to that later). I applied via email and sent my resume as the listing had said. The next day the area hiring manager emailed me and asked me to schedule and interview and I did. I went to my interview which I had scheduled for early in the morning. When I got there we were given the application to fill out. In about 15 minutes me and the two other interviewees were called into the office for a group interview. The interviewer said that the company does group interviews because it can better show group dynamic and how you interact with other people in a team setting and what have you. So we got into the interview and the interviewer asked us each to tell the group something about themselves. We each gave our name, age and a little anecdote about ourselves. After that he began to ask us each questions and then the next person would build upon the answer in most cases. Some of the questions were:
Where do you see yourself in 5 years?
How do you feel when people lie to you?
What motivates you?
Would you rather work alone or with others?
What is the benefit to working alone? With Others?
What did you learn about Jane Doe? (Person interviewing with you.)
How would you handle a hostile customer?
What do customers want from sales associates?
After all the questions between the interviewees, we were asked to sell something on the spot. What I mean by that, is that the interviewer looks at something in the office and just tells you to sell it to him. He said that he does this to see if you can think on your feet and can improv sales tactics and talk to customers. After that we were shown around the clinic so that we could get a feel for the atmosphere. Where all was said and done, he asked if we had any questions and one of the questions was asking about the pay. We were only told that the pay starts at $8 and if we make our quota in the 1st 30 days we would be raised to $10. What happened to the $9-$13 quoted in the ad?
Within a week I recieved another email to go on a second interview where you would shadow someone to see how the sales scripting would be done and to see how a normal day to day operation would go. I declined to further pursue the opportunity because I was offered a job at another company starting at $10 with a $0.40 raise after 30 days. Better than nothing right?

Reason for Declining
I declined the offer because I was offered another job before I recieved the email to go on the 2nd interview. The job I am with now is not based on sales and I still get $10/hr with quarterly raises.
