You have to pull them to the side and let them know that they're not doing well and inquire as to what they think the problem could be. Allowing them to explain their side of the story not only provides you with a complete answer, but it can allow you to learn things from their perspective. Having both perspectives will allow you to come up with a strong solution so that the person can perform appropriately, as well as to not waste time for the company.
