The PhD program director in Neuroscience contacted me initially to offer me an interview. The MD/PhD program application process is done mainly through whichever PhD program you're interviewing for. The medical school interview process is integrated into your PhD interviews. Thus, out of 3 days interviewing, half of a day was exclusively for medical school. As will any interview process, you must be able to describe yourself well, why you want to commit to such a long program, and explain everything professionally.

They'll offer you a normal graduate student stipend with a full tuition waiver. You can always apply for exterior fellowships through the NIH and NSF.
