One of the most important things to do before you have this conversation is to write out a list of the issues with specific examples supporting your statements. Review it. Ensure it is correct with no ambiguities. Rehearse the conversation. Understand the possible outcomes and what your plan is after the issue has been presented to the employee.

Before you have the conversation review it with an HR rep within your company to ensure compliance to policies, etc.

Finally, above everything else, never allow the conversation to stray into the blame game. The associate will try to blame others, and possibly you. Remember, this is about them and no one else.
