    I worked at Geisinger Health System

    Pros

    Some real estate in the area is cheaper than say California or other high cost areas. A few departments here are run by people who try to make a fair system despite the overall problems. There are a few excellent physicians here.

    Cons

    Attendings, think twice about accepting employment at Geisinger until physicians are treated better. You might find a reasonable department, but you will have to perform highly (in $ only) to stay here.

    Attending physicians at Geisinger are given monthly sheets showing target RVU (financial) goals versus actual achieved financial goals. Physicians who do not meet financial targets continually, even if they accomplish much for patients, are encouraged to leave.

    Here are problems with how Geisinger sets attending physicians up:

    1) Monthly RVU sheet encourages a run up patient tests and procedures (generally on those who have other than geisinger insurance) to increase the RVU numbers
    2) If accomplishments are medical for patients, and not always financial, they are encouraged to leave as mentioned.
    3) There are many doctors at Geisinger that game the RVU system. Basically they order the expensive tests/procedures so the RVU then goes to their name, where-as, the complex time consuming history, diagnosis, paperwork the RVU gamer physicians just refer the patient to another geisinger attending physician who will get lower RVU from these. This is done OFTEN, many physicians here have learned to do this to look better on the RVU sheet for potential bonus, etc!
    4) No official time is given to enter material into electronic medical record, so it is all on the physicians own personal time
    5) The 30 minute lunch is your only given time to follow-up on patient phone calls, so good luck eating and talking.
    6) Physicians are not given time to see their own physicians/dentists even for annual checkups. Even if you schedule one, they stuff the schedule full so you can’t go.
    7) Physicians often made to work over 100 hours per week if you include paperwork
    8) Are not even provided things like light bulbs, pens, or extension cords in offices, and they must provide their own.
    9) Are routinely sent angry emails by non-medical administrative staff.
    10) Often not allowed to take holidays or vacations other than a day here and there.

    Each department at Geisinger has taken to hire extensive and highly paid management (administrative doctors). These are often two or three physicians per department who could not land such a job elsewhere because of their qualifications. These few and highly elevated are used as a populist happy appearing front to others, whereas the main physicians who do the bulk of work are left unhappy

    Not only the above but Geisinger in other locations than Danville actually loses money. They make money in Danville because they own the town essentially - the old Carnegie model of business basically. Control people, control the lawyers, and control the area.

    Advice to ManagementAdvice

    Do not emphasize money so incredibly much. Treat physicians as a true asset not something to exploit.
