Management have contempt for employees
Promotion and remuneration are based on personal relationships with managers
Poor technology and mis-investment in it
A large number of very poor quality people
Extreme amounts of red-tape

Advice to Senior Management

Learn what people actually do in their jobs rather than assuming that you know and then telling employees the solution. Identify and eliminate crony-ism and either introduce a real meritocracy or stop saying that the business is one. Stop throwing money at technology problems; with endless re-architecture programs that never actually go anywhere.
