Lack of Sr. Management planning
Lack of Sr. Management understanding of what it takes to complete tasks
Environment of knee jerk reactions

Sr. management is flat, focuses on individual task list verse team approach and input.
Advice to Senior Management – Listen to risk avoidance advice from staff
Listen to experienced employees timelines (it is based on experience for a reason)
Be less secretive and work with vendors verses trying to dominate
