Fast turnover in some markets due to difficulty securing engagements and lack of opportunities
Occasional negative workplace politics and micromanagement
Lack of proper channels for individuals to vent about frustrations with being seriously punished.
Punishment for creativity/ambition/intelligence
Rewards for falling in line, tolerating mistreatment, and refusing to utilize professional skepticism
Creative minds are sometimes pushed out and autocratic egomaniacs are encouraged
Occasional racism, sexism, poor treatment of those lower on the pecking order by superiors
Bureaucracy...Who you know is everything and what you know is best kept to yourself

Advice to ManagementAdvice

Treat your newcomers like gold, as they will someday be the future leaders of the company.
Offer a more honest and realistic feedback loop, rather than telling people what they want to hear while making side judgments and preventing your employees from actually bettering themselves.
Fix the tone at the top to emphasize equality, respect and fair treatment of everyone in the company, instead of encouraging new consultants to put up with everyone else's crap until they can someday dish it out and perpetuate the cycle.
Actually punish those who have a horrible attitude and verbally abuse those below them, and reward those that are ambitious, creative, and get their work done faster than expected.
Don't focus so much on getting hordes of new employees through the door and then cutting the chaff, spend more time on finding employees who are a better organizational fit for the company and are successful with interpersonal interaction, and you will greatly reduce company turnover.
Don't allow management to blame individuals for failures on the part of management, the company, or factors that are completely out of everyone's control.
Have more realistic expectations of your workforce. With unrealistic expectations, the stress builds up and is the worst for those at the very bottom. The levels of anxiety resulting from unrealistic expectations, poor communication, and blaming the victim are paralyzing and seriously hinder productivity.
