Job Description

We are currently seeking two top-notch individuals to serve as a Registered Nurse (Case Manager) for the LaSalle & Peru IL area; and the Rockford area. The idea candidates would live in these service areas.

​​POSITION SUMMARY: The Registered Nurse (Care Manager) is responsible for the supervision of all direct care services and serves as the leader of a Patient Care Team in the assigned area. The Registered Nurse (Care Manager) is also responsible for supervising, evaluating and coordinating the various members of the Interdisciplinary Team; serves as the patient advocate and coordinator for other social service and health care providers in the community who are involved in the care of the patients; and assumes responsibility for the maintenance of patient records from intake to discharge or transfer to bereavement team.

ESSENTIAL FUNCTIONS:

  1. Completes an initial and comprehensive assessment of the patient and family to determine hospice needs.
  2. Provides a complete physical assessment and history of current and previous illness (es).
  3. Provides professional nursing care by utilizing all elements of nursing process.
  4. Assesses and evaluates patient’s status by:
  • Writing and initializing plan of care.
  • Regularly re-evaluating patient and family/caregiver needs.
  • Revising the plan of care as necessary.
  1. Initiates the plan of care and makes necessary revisions as patient status and needs change.
  2. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions when appropriate; includes the patient and the family in the planning process.
  3. Administers medications as prescribed by the physician in the patient’s plan of care.
  4. Provides education and instructions to the patient/family as appropriate per assessment and care plan.
  5. Acts as Case Manager when assigned by Assistant Nursing Director or Regional Nursing Director and assumes responsibility to coordinate patient care for assigned caseload.
10.Completes, maintains and documents accurate and relevant clinical notes regarding patient’s condition and care given in the electronic medical record system. Records pain/symptom management changes/outcomes as appropriate.

11.Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders are required.

12.Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician.

13. Provides and maintains a safe environment for the patient.

14.Assist the patient and family/caregiver and other team members in providing continuity of care.

15.Works in cooperation with the family/caregiver and other Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.

16.Attends IDT’s as assigned.

17.Participates in on-call duties as directed by the Assistant Nursing Director or the Regional Nursing Director.

18.Ensures that the appropriate DME equipment and other services are in place in accordance to the Plan of Care.

19.Supervises the Licensed Practical Nurses and Certified Nurse Assistants per the patient Plan of Care and delegates’ responsibilities when required.

20.Assumes responsibility for personal growth/development and participation in continuing education and in-service trainings.

21.Conducts assessment on new patient referral as directed by the Assistant Nursing Director/Regional Nursing Director; and/or accepted new case assignments.

22.Other duties as assigned.

REQUIREMENTS:

  1. Graduate of accredited school of nursing; BSN preferred.
  2. Current RN License in the state of employment.
  3. One year recent nursing experience in acute, long-term care or homecare/hospice/public health.
  4. Patient care planning and patient teaching skills.
  5. Computer Literacy.
  6. Current drivers’ license, valid vehicle insurance, and access to a dependable vehicle for daily travel
KNOWLEDGE, SKILLS AND ABILITIES:

  1. Ability to recognize the needs of people and maintain a good relationship with them, have
experience in the areas of grief and loss with the ability to recognize and relate to the problems

of grieving families.

  1. Excellent communication skills both verbally and written.
  2. Ability to demonstrate problem-solving and organizational skills.
  3. Ability to effectively present information in one-on-one and small group situations to
patients/families/caregivers, and other employees of Passages Hospice.

  1. Excellent documentation and assessment skills.
  2. Ability to work within an interdisciplinary team setting.
  3. Ability to make meaningful observations, write brief and accurate notes.
  4. Ability to maintain confidentiality of health information (e.g., Privacy Act, Freedom of Information Act, Health Insurance Portability and Accountability Act (HIPAA) and HITECH Guidelines.
