Regence BCBS. Two options - standard and high deductible. On standard you don't pay for employee premium. On high deductible they contribute to HSA so you kind of make money.
Not sure how typical this is, but our insurance makes us jump through a bunch of hoops to get them to pay for anything. Almost everything gets denied initially and we have to deal with a bunch of headaches before we can get it approved.
Surprisingly good for a small company. Reasonable PPO option and it's taken just about everywhere - never had an issue choosing a provider and the cost for out of pocket has been reasonable for everything including child birth.
