401 K match is 1%. As a therapist, you get 6 days of PTO for the year and 2 paid personal days. You do get paid holidays - I believe there were 5 this last year. If you leave the company, you do not recieve any pay for your accrued time. You must work 90 days to start using PTO.
They are great benefits, you have to contribute to spouse/dependent benefits, but yours are almost all covered.

Good insurance for department heads who can afford it. The insurance that is offered to the floor staff, i.e. housekeeping/CNAs is not good or worth it

Great compensation, health and retirement package. Comparative companies come close but Cantex has a reputation of being the best in this area. Makes for a stable foundation in planning finances
If you are not management, you get the crap benefits. You cannot use sick or vacation time until you are employed for six months yet turn over rate at 3 months is 95% or more so no time off that is paid really except you one "free" day aka your "personal" day. If you manage to survive a year, they take most all of your sick time and place into a medical leave bank that you can only access if you go on FMLA and if approved by corporate HR to take the time. If you leave with good standing they still don't pay out your sick/vacation time like most other places do. Just read your handbook and you'll see just how confusing their policies are, all geared to only work in the company's favor.
Vacation accrual and sick time accrual is average. 401K is optional. Health insurance employee contribution can be expensive but there are three different options including a HSA.
