CareFirst BlueCross BlueShield is accepted almost everywhere. It's convenient and works for my lifestyle. Most doctors accept this insurance plan.
Best: Receive up to $1000 each year for meeting basic wellness qualifications (not smoking, low BMI/in weight management program, flu shot, having a primary care physician, etc) Worst: Maternity policy is simply short term disability. If less than 5 years at the company, then employees do not receive 100% salary when out on STD (only 66%).
