One of the worst insurance programs I've ever been a part of.
It was expensive but gave a good option of providers.
HMO option with copays, or PPO HSA with HSA cash paid by the employer to your account.
High deductible, but they deposit into your HSA to make it easier.
It used to be great. It's now awful. High Deductible Plan with a huge premium payment I paid like $300 per paycheck to have insurance that had a $5,800 out of pocket max for a family. It used to be copay plan with no deductible and a lower premium.
