We had "unlimited" Paid time off which equated to a maximum of 20 days off. Many people lost time/money due to the new plan. It was extremely difficult to administer and you were not paid for up to a week's worth of time for any time that was unused under the previous plan.

Grandfathered in to 17 days/year. This includes vacation and sick days. In some practices, if you are on the bench they take your vacation days. They took away our ability to cash out 80 hours/year. The new policy is "unlimited" PTO with restrictions. It was not, of course, of benefit to the workers.

Average paid time off, but good opportunities to take leave without pay.
