You are eligible to participate in the Company’s benefits programs if you are a regular full-time employee working at least 30 hours per week. Benefits, unless stated otherwise, are available on the first of the month following one month of employment.
The medical, dental, and vision plans offer you the ability to cover yourself only, or yourself and your dependents (including domestic partners). Note that your dependents can only be enrolled in the same plans you choose for yourself.

Would be nice if benefits were 100% paid, but still better than a lot of other companies
The cost increase for 2019 was very high for the PPO — unfair to people who need the flexibility of a PPO.
The health insurance I chose does have a high deductible, but a very reasonable low premium. Cotiviti offers an option of an HSA card, which they will match a generous amount to as well.

Overall benefits package is fair and comparable to the market. HSA is a great plan for those who are health conscious and responsible, but it may not be for everyone.
Expensive, no real choice - either high deductible or traditional
The cheapest I had ever had throughout my career life. Lots of options.
