Great benefits at a decent cost. The retirement is not a match but a contribution whether you participate or not.
Health, dental and vision insurance, FSA, HSA accounts, life and disability insurance, 401k, opportunities to reduce your health ins premiums, however, I have found employee insurance premiums to be higher here than most places
Health insurance is too high, especially since they are a healthcare provider. I had better benefits working in a call center. They will not let you use your PTO when you want to and they ask specifically why you need to use it. They violate their own policy by doing this. They are not putting the money they are suppose to be putting in your 401K. They are actually being sued for this by a former employee who retired only to find out her 401K was empty. They are very dishonest as a company.
Best: If you happen to use OLOL doctors, benefits are paid at 95% for services through OLOL physicians. Worst: If you do not use OLOL doctors - my out-of-pocket expenses were extremely high compared to my old employer benefits (which lessened the amount of increase I received for changing jobs), THEN on top of this, micro-managing with unrealistic work load views is too common in this workplace!
Benefits aren't really that great, not terrible... They are self insured, so you have to jump through hoops to get any prescriptions approved. You get less standard holidays than most employers usually offer, the insurance is expensive. One of the most archaic (old-fashioned) employers i've ever worked for... It's 2016, wake up sheeple!

Health Insurance is not customer oriented. Current insurance does not pay as well as several years ago. Time earned for time off has decreased per pay period.
FMOLHS touts their package as being very competitive whereas a diligent level of research will show a person that the package is actually a little below average. The best thing about the package as far as PTO is that FMOLHS is rather "loose" about how PTO is used. Persons are not charged PTO if they leave early or need to come in late due to a doctors appt or something. The basic philosophy is that they trust you to put in what you take out. This is a nice perk.
