The company offers good PTO accrual but starts at 3 months from hire date.

200 hrs a year has to cover vacation, sick leave and holiday pay. Nothing above average about this benefit.

I have never worked at a company that give the amount of PTO that they give here. 4 weeks to start and them 5 weeks after 4 years.

out of your bank for each holiday in order to be paid for the holiday. If you don't have enough hours in your bank to cover the holiday, they deduct the hours/pay from your paycheck for that pay period (even if you are salaried), so you could end up with a smaller paycheck for that pay period. I have never worked at a company that didn't offer paid holidays independently of accumulating paid time off for vacation or sick time. But hey, even if we get docked some pay for Christmas, they do give us a party.

Vacation, sick time, and holiday time come out of one bank of hours. Even if you are salaried, you have to take 8 hours out of your bank for each holiday in order to be paid for the holiday. If you don't have enough hours in your bank to cover the holiday, they deduct the hours/pay from your paycheck for that pay period (even if you are salaried), so you could end up with a smaller paycheck for that pay period.
