For salary employees in SE Michigan: Two health insurance plan options, coverage is the same. Only difference is pay lower premiums with higher out of pocket max or higher premiums where out of pocket max == deductible. Pricing for single/two party/family with lower premiums Monthly Premium: $40/$80/$120 Deductible: $1500/$3000/$3500 OOPM: $2300/$4600/$5600 Dental: $18/$36/$54 Vision: $8/$16/$24 Plan is BCBSM PPO, lots of provider options. Nurse hotline and online doctor visit options are available for convenience and sometimes for cost saving. Company gives you $1500 in your HSA if you (and spouse if applicable) get a physical each year. HSA is through BoA and has a lot of investment options, some low fee and some higher fee. In 401k, company gives you 4% contribution no matter what, then matches 100% of your contributions up to 4%. So while a 4% match doesn't sound like much, its actually 8%. Good plan through Fidelity with low fee funds to choose from. Pretax, roth, and after tax options all available. Can mix and match as desired. Vacation starts at 15 days, goes up to 20 by year 3, but 4 of those days every year are mandatory to use during the week of July 4th when company is on shutdown (whole week is off but July 4th is paid holiday instead of vacation day). Company also shuts down from a few days before Christmas until Jan 2 every year, this shutdown doesn't take your vacation days, varies from 6-8 work days depending on what day of the week Christmas falls. In addition to July 4th and winter shutdown, there are 8 paid holidays. Election day is an additional paid holiday during election years. Vacation days are use-it-or-lose-it and reset on Jan 1 each year. All vacation days are available on Jan 1, no accrual system. Employees are provided a smartphone by the company (choose iPhone or Samsung). Many people choose to use this phone plus Google Voice as their only phone and stop having a separate personal phone. Many locations have a gym facility on-site that is free and open to employees of that site. Size and quality of the gyms varies.

Spiff programs on weekends are exciting, and a decent way to bring home a few hundred more a week. Their 401k match is atrocious. Health benefits are expensive and not very competitive.
