    I've used it plenty, and have had no complaints. It's not cheap, but it is affordable, and the care seems to cover everything I need

    Family rates are about 250 per pay period. It includes medical dental and vision. They will deny claims and some meds that are necessary so you often have to appeal and keep an eye on things.

    Aetna. they take a lot out of your paycheck. not worth if, if your spouse has insurance I highly recommend taking theirs (most employees here take their spouse's insurance instead of from icon).
