We have an excellent 401 k plan. The company matches 100% of employee's contribution up to 4% of salary and then matches 50 % of the next 2% of salary.

Company matches your 401k up to 6%. The first 4% is fully matches. The additional 2% is matched 50%. The health insurance is not great but is it better than a lot of our plans. There are yearly prescription deductibles of $50-80. Medical coverage deductible is around $2000. Copay ranges from $20-35.
Infinity kept the cost to employees reasonable despite the enactment of the ACA. Insurance add-ons like additional long term disability, vision and dental insurance were affordable.
They had nice employee benefits. Offered 401K, health benefits, dental, vision. The paid vacation was ok. Had 1 week paid and had to earn any additional vacation per paycheck. Work from home position was a perk
They offered medical, dental, vision, and life insurance. The prices were comfortable. Also, the offered 401k and the matched your contribution. Overall they have a great benefit package.
The company pays for a good amount of the cost of the insurance benefits, the only con is that the benefits are 80/20 and the out of pocket expense can be high if the medical costs were high.
