ISU offers two health plans, both through Wellmark. One is a PPO and the other is an HMO. Both have relatively low premiums and max-out-of-pocket compared to other plans I have had at other employers.

The PPO plan has no cost for just the employee. Adding a spouse is only $78 per month and adding children is $115 per month. Co-Pays are typically $10 each visit.
