401k match was cut in half in 2014. You have to contribute 8% to get the full match of 12%, and they pay out their match quarterly instead of monthly so if you are let go before your quarter is finished you get zero match for that quarter.

The health package is typical for this day and age. Company heavily pushed HSA plan but did subsidize the plan if you met certain goals. Vacation and time off was satisfactory, however, there was a limit of 80 hours that could be rolled over. This was a big issue because deadlines often made taking vacation difficult. Work from home was available to some but it really depended on your relationship with your manager. If he/she liked you it was fine, otherwise it was discouraged.
