The benefits package is horrible. There are no company perks unless you want to buy a house. Insurance hardly covers anything-it cost me $120 for a general doctor visit. Only 10 days PTO.
There are 3 tiered options you can select from for health insurance, but all three have a large deductible. There is an HSA and FSA offered. Medical, dental and vision is all offered. No 401K match, but a profit sharing amount is deposited at the end of the year into your 401K. This varies depending on how well the company did, but it isn't very much. Paid time off is all inclusive, which means if you get sick, you have to take a PTO day. 15 days with a floating holiday if you have been there for a full year. They do NOT carry over, so you must "use them or lose them".

Have PTO. Have only 8 paid Holidays a year. Only Christmas Day off. Insurance is relatively inexpensive but high deductible no copay.
Nice balance of PTO, 401k and stock purchase option.
