Solid PPO with BCBS, good dental and eye care.
Medical: MAXIM has a wide variety of BCBS plans, from high deductible (lower paycheck deduction) to low deductibles. Also Kaiser which I have not used. If you select high deductible plan, you can put $ aside in an account to pay your deductible later. Dental: I am relatively happy, but only comment about dental is usually the coverage for out of network dentists are pretty low. Which makes your visits expensive. 401K matching is good.
