Worst was the reduction in the Company Match over the years. Many, many, many different changes over my the course of my time at that company.
They offered a 4% match on the 401k plan.
It goes down in value when the economy is strong, and then it goes down in value when the economy is weak.
Its a half percent match per every percent the employee puts in to a max of 5%

The investment options are ok. Several index funds and blended stock/bond "date" funds. There is a quarterly fee charged by the plan administrator, Fidelity. You should at least consider contributing anything about the amount required to max out your company match to a separate IRA.
NCR's 401(k) plan is average when compared to other plans in which I have participated.

They only contribute the company portion at the end of the year.
They require a 10% contribution for employees to achieve a standard 100% match of 5% contribution, since they match only 50% of the contribution. The match is also withheld until the 1st of the next year, so if your employment is terminated prior to 12/31 of the current year, you lose all of the company match.
