A couple of PPO options, and a high deductible with HSA option. The company completely covers the cost of high deductible for employee and family.

Good, comprehensive care. Typical problem with HSAs and high deductible. If I am an executive making $200K/year, a $2000 deductible is fine, but what about the person who makes only $50K/year?
Self-Insured, Managed by BC/BS. Very accommodating. Virtual options. Overall easy to work with. Some challenges finding referral partners for Doctors but in all very happy.
The healthcare premiums are now all paid for. Saves you money, especially if you have a family.
