Saved us from going bankrupt from medical expenses. When insurance stopped paying the mine helped to pick up the tab.
Several options, including that of HSA. The PPO is kind of pricey for family rate and it's not based upon salary.
HSA or PPE plan, both require a monthly fee. I never had a need to use the HSA, but I still have my HSA account with two years of annual contributions from the company.

Company subsidizes health care plan, but only for employee. Rates for spouses or family are much higher. Company does contribute to employee's HSA plan, regardless of employee's contributions.
