Health insurance is expensive, but company recently changed health insurance which reduced the cost a little. Wish there were stock options.
Overall it's expensive but the company does actively TRY to find ways to improve the cost (with limited success).
Medical benefits. They run from $200-$400 a month for a high deductible plan. Company does not contribute to 401K til you're about 5 years in. You only get 12 PTO days, which includes sick days, personal days and vacation days.
Worst benefits I've heard of in this industry: slim to none of health insurance was covered by company (almost $130/month), you receive 10 business days vacation (this is very strict)

Health Insurance was standard, 401 K was matching up to a certain percentage (maybe 3.5%?, ie. not great) and vacation was 2 weeks with more to be earned the longer you were an employee or role in company changed. Some locations offered occasional work from home days (pending your manager)
No 401K matching. Health insurance options are expensive.
Car & Phone Allowance. Monthly bonus is you meet your sales quota.
