Employees are not allowed any vacation in the fourth quarter. Lots of people complained about the lone health insurance plan.

Ultra high deductible for both family and single with semi-reasonable monthly costs (if the deductible was not so high). Offer HSA and FSA with good 401k match. Vacation and PTO are similar with most companies.
Overall, the benefits are worse than you can receive elsewhere.
Health benefit cost were reasonable considering the skyrocketing cost nationwide; in general there were two to three plan options from which to choose. Company matched 401k contributions 50% on the dollar up to 8% of your income (this is above market average).
