You can pick almost any doctor/specialty without going to PCP first, and family of 3 or more pay the same after 3 people
It is good, but expensive! Employee pay is around $500 - 600 per month.

This maybe be a 3.5 stars. High deductibles and OK, but not great plans. The wellness plan can help offset a lot of the monthly costs, but you're still paying for the deductibles and out of pocket costs.

SCJ contributes 1500 for health care. There is an annual benefit for bio-metric data, HDHP costs 1500, LDHP is free assuming "perfect" health.
