Unlimited PTO was a lie. It's totally up to your manager. Your manager can say no any time as you did not accrue anything. I found people who have longer tenure or already "in the club" can take ridiculous amount of time off. However, new people might be in trouble for taking "approved" PTO. So before you join, take your break and make sure you don't have errands to run for the first 6 months of so to leave a good impression to the management.

My Vacation was refused during Christmas. And I was asked to check in fix for test failures / fixing flappers for code that was documented by a Junior Team Member 2 years back. How was that checked in at the very first place - redundant test got code reviewed.

Roughly 3 weeks per year, but as long as you meet your utilization targets, it's not really a fixed number. With a little notice to your manager and arrangement to cover accounts, it's never been an issue.

ServiceNow has the "unlimited" time-off policy which is fine if you can find the right time to leave.

3-4 weeks for Vacation & Paid Time off, but it is completely depending on manager approval. Along with these there will be a good amount of holiday's for Thanks Giving, Christmas and New Year.
