Expensive insurance that is unaffordable. 401k with no match. PTO is not kept upon leaving. They were bad when I was hired, and they got changed for the worse while I was there.
Not good, didn't even pay out PTO when left. Never got to even use any because understaffed constantly.

Salary Managers work insane hours that should be illegal.
Terrible benefits. High monthly cost and the lowest deductible on a plan was $2500. Garbage benefits.

Benefits start after 60 days instead of the typical 90. They just started a 401k plan for 2016. Vacation day/sick days max at 5 weeks. They have many options for selecting health insurance, dental, disability (this one is a bit pricey). You get two free pairs of glasses each year, one on the first day and the second after 60 days. Associate discounts for glasses & contacts are cost +20%! Friends and family get cost + 50%. That's the best discount I've ever seen in this business.

Health insurance was pretty standard. we got a reasonable amount of time off but rarely had the opportunity to use it
