Amazing Benefits with low deductibles
The best is it is meeting the average expectation of health, dental and vision benefit. The worst is the 401K. 1. match is low. 2. doesn't allow after tax 401k contribution.
Medical Insurance - Premera BlueCross PPO plan with 100% coverage for a preferred provider and 70-100% coverage (depending on the benefit) for participating provider. Office visit co-pay is $15. Annual deductible: $200 individual/$600 family. Employee Only - $0.00/mo Employee, Spouse and Children - $222.00/mo Dental Insurance - Delta Dental PPO plan with 100% coverage on preventive procedures. $15 co-pay for basic procedures with 90% coverage and 60% on major procedures. Orthodontia coverage for children and adults in the amount of $2,000 per person. Employee Only - $0.00/mo Employee, Spouse and Children - $18/mo Vision Insurance - Premera BlueCross Contacts, prescription lenses and frames covered up to $300.00 each calendar year. Company bonus is tricky math which is not so appealing, stock option was not great when compare with other companies, Health benefits are ok.
