I think the cost of the health, dental, and vision insurance plans is very fair and has good network coverage (compared to what I use to pay working at a different company). For my family of four, this year I'm paying $71 per paycheck for health, $37.60 for dental, and $11.90 for vision. Other benefits offered (401k, life insurance, disability, time off, discounts) are also good. Company also has several wellness initiatives and events that I think are an added bonus and make me feel like they care for my well-being. They truly try to make one of our core values happen - Be Well, Work Well, Live Well.
Under the "plan d" or High deductible savings plan, the company is willing to add money to my personal HSA. The deductible is fairly low for being an HDHP compared to other plans that I have seen, and the cost per paycheck as a single is very reasonable. Dental and vision is also offered. Insurance is self funded, and rates have been pretty stagnant (huge benefit) during the duration of me working for the company.

15 days vacation on hire, then 20 year 2-5 and 25 5+. 401k match up to 4% but not fully vested for 5 years. 3 health insurance plans through UMR (second brand of UHC), deductible ranges between $750-$2500. Dental and vision insurance are available for purchase. You get 1.5x current salary life insurance provided. 10% discount on FunJet. You can buy a IATAN card for other discounts for $50 a year. No bonus, profit sharing, or stock options. Work from home depends on how much your manager likes you, no clear policy on that. No free coffee or snacks regularly. Coffee is .75 per cup and soda is $1.67 per bottle in the basement break room. Break room has prison style tables and chairs. No BYOD phone option unless you are very high in the company. Tuition reimbursement of $350 per class if you get an A.
Benefits are exceptional and above the average from what I hear. Health insurance cost is very reasonable, 401K company matching is great, vacation/illness/personal paid time off is quite generous, free life insurance is a really nice perk. An added perk the company should consider, and would align with their wellness initiatives, is a free membership for employees at the nearby Wisconsin Athletic Club (or similar).
