We we're acquired by Concentra earlier this year. Some benefits have rolled over to Concentra, others will begin when the full transition occurs.

401k with company contributions Terribly priced healthcare - extremely expensive Limited maternity leave (6 weeks) Limited time off that accrues over time

Unless you are gravely ill or had a surgery, you will never use the healthcare benefits. There is a fairly large amount taken out of your paycheck and high deductible planes.

401k low matching, vacation and sick leave standard 2 weeks vacation, 1 week sick leave. The company has a use it or lose it vacation model where you are unable to carry over vacation time making it difficult to take extended trips.

U.S Healthworks offers health, dental and vision insurance. Also 401K, short term disability, FMLA, maternity leave. Benefits are pretty reasonable.
