See above breakdown. I think it's a pretty solid plan.
3-weeks vacation when I started. 4 weeks shortly after (company wide). Unlimited by the time I left.
Volusion just introduced unlimited PTO for salaried employees and 5 weeks of PTO for hourly employees (up from 4 weeks).

3 weeks, plus 2 days, plus birthday, plus holidays to start. After 5 years, add another week. After 10 years, add another week.
Minimum PTO is 4 weeks.
They raised it to three weeks a year PTO, that includes your sick time.
Vacation time is accrued at a fair rate. They also provide two floating holidays that can be used anywhere. They have bereavement leave which doesn't draw against your regular PTO in case of a death in the family. You also get your birthday off! Sometimes as a perk, they company rewards employees with more days off based on tenure and hard work/goals reached.
