Clinical case managers ensure patient or client needs are understood clearly, and that patient care is safe, timely, and practical with a client-centered approach. They draft service plans, review case progress, and determine case closure. Clinical case managers help clients achieve wellness and autonomy and make informed decisions by acting as advocates regarding their status and treatment options. They address motivational and psychosocial issues and promote quality and cost effective interventions and outcomes. Clinical case managers need a bachelor's degree in healthcare in a healthcare related field and a proven working experience in case management.
