What does a Clinical Case Manager do?

Clinical case managers consult with patients and their families and health care providers to coordinate and ensure that the treatment and care plan in place is carried out properly and brings the desired results. Clinical case managers coordinate tasks between doctors, insurance companies, rehabilitation, and home health care. They assess patients and oversee their complete cases and coordinate with the money providers, including insurance companies, involved in their care.

Clinical case managers ensure patient or client needs are understood clearly, and that patient care is safe, timely, and practical with a client-centered approach. They draft service plans, review case progress, and determine case closure. Clinical case managers help clients achieve wellness and autonomy and make informed decisions by acting as advocates regarding their status and treatment options. They address motivational and psychosocial issues and promote quality and cost effective interventions and outcomes. Clinical case managers need a bachelor's degree in healthcare in a healthcare related field and a proven working experience in case management.
