CODAC envisions a society where everyone lives healthy, productive and caring lives — positively contributing to their communities.
It is the Mission of CODAC to provide tools, support and services to individuals ...
CODAC is one of Arizona’s oldest and most respected community providers of specialty care for mental illness, addiction and trauma. What began as a grass roots drug abuse prevention program has grown into a multi-faceted organization that provides services across the entire spectrum of behavioral health care.
Through 20 service locations and over 450 staff and volunteers, CODAC now serves more than 12,000 individuals and families each year. CODAC is a nonprofit 501(C)(3) organization.
We are here to help you meet your mental health, substance abuse, trauma, primary care and general wellness goals. A variety of treatment professionals will partner with you to develop a plan and take positive steps toward a healthy, productive, caring life in recovery from a mental illness, substance use disorder and/or trauma.
In addition to our behavioral health care services, we now offer primary care, allowing us to care for individuals’ and families’ whole health care needs. We understand the needs of individuals with mental health and addiction disorders. Mental health affects physical health. And, your physical health can affect your mental health.
CODAC can help with common illnesses, injuries, physical exams and health screenings. You can take care of lab work, fill prescriptions, get your wellness checkups, as well as other chronic conditions such as diabetes and high blood pressure.
A team of experts – primary care providers, along with psychiatrists, nurses, medical assistants, therapists, case managers, and peer support specialists– work as one team in one convenient location. CODAC is focused on physical and mental health, giving our patients the most complete health care available.
CODAC is your home for health care.
