We have more than 20 years of experience working with payors and providers to create programs that improve quality and lower costs by allowing patients to age and heal where they want to be: at home. We currently manage care for 24 million members through over 8,000 provider locations.
Through advanced analytics, we determine the appropriate site for post-acute care, and provide support and coordination for patients and their families throughout care transitions, including to and from Skilled Nursing Facilities (SNFs) and through Home Health, Home DME, Home Infusion, and Home Sleep services.
We engage patients and caregivers, coordinate care transitions, improve clinical outcomes, and do all we can to help patients heal and age at home.

I applied online. The process took 2 weeks. I interviewed at CareCentrix.
I received a call from HR and was immediately asked what my salary expectations were. The HR rep then scheduled another time for the two of us to talk. She called me a half an hour late and had a five minute conversation with me to set up another interview time with a manager. I then talked to a manager and he asked me typical questions. Then two days later I talked to another statistical analyst. I felt both interviews went well, but had a very hard time get through to them afterwards. I eventually heard that they were still considering candidates. I'm assuming I won't get hired, though they never formally said that.
