Community Health Associates – Why Work With Us?

We are licensed by the Arizona Department of Health Services, Office of Behavioral Health Licensure to provide outpatient behavioral health treatment services. We are also a provider of Certified DUI Screening, Education, and Treatment services, and Certified Misdemeanor Offender Domestic Violence Counseling services. CHA is a Certified AHCCCS provider and a contracted network provider for Cenpatico Behavioral Health Services of Arizona.

Referrals to our treatment programs may come from physicians, schools, social service agencies, courts, and through self referrals.

We are committed to delivering mental health treatment services to consumers/clients by providing services in the least restrictive environment and in locations where the consumer/client is most comfortable. The goal of each service is to assist the consumer/client in achieving optimal functioning in their daily lives.

Crisis support services are provided 24 hours-a-day, seven days a week.

Each individual's treatment process is guided and monitored by a multi-disciplinary staff.

All aspects of service delivery are monitored to assure quality treatment and ethical business practices.

Our philosophy of care is dedicated to helping improve the quality of lives of children, adults, and families, and the communities they live in, by building upon strengths. The Recovery Model is our standard. Empowered recovery is the backbone of what we do. We believe that "empowered recovery" comes from the individual and means that our staff are committed to helping individuals and families learn how they can fully manage their mental health illness or emotional and behavioral health problems. We hope that you find your visit to our website helpful. Please use our Contact page if we may be of assistance to you.

Our philosophy supports every consumer's right to receive care in the least restrictive setting, to live independently when possible, to be employed and to be educated. Our staff are committed to helping consumers fully exercise their rights, and learn how to use local resources, and systems of care, to meet individual and family needs. We also believe each our clients and consumers must have decision-making authority and "choice" regarding their care. Respect must be mutual, with an understanding that Recovery is a life-long process and that success requires support and involvement of staff, family members, and school and community representatives.

We are always taking applications for qualified candidates. Our workforce consists of full and part-time employees, and schedules vary according to the position. Our hiring needs also vary during the year. For more information, please contact the HR Recruiting Coordinator, Jack Kolodziejski, at 928-376-0026, ext. 1149 or by email at
