Lack of checks and balances. Meetings with no agenda, minutes or objectives. Heads who do not know how to manage processes, are not accountable.
Advice to Management
Keep a tight leash on heads who are obviously either naive or corrupt. Management seems to be easily misled by the actions of a few non-performers.
