If the previous year was very good to great and the current year business is lackluster: a 20% goal increase can be tough to reach and thus kills commission attainment. No adjustments once set. Working hard & smart & skillfully only goes so far...

Quarter by quarter review to verify realistic goals.
I applied online. I interviewed at Sterling Financial (WA) (Boston, MA) in February 2013.
Had two interviews. Both within approx 2 weeks of eachother. Pretty standard, was sent an email. Met with the VP and HR. Then met two of the sales reps I was going to support.
