Systems are made and changed as you go along. There is a vision, but the emphasis was always on getting things done "somehow" and not with sustainability in mind
Highly people-dependent, political system.
Disconnect between line managers and HO managers
Advice to Management
Bring in more robust systems which are people-independent. Take teams into confidence instead of acting as if this is a war in which the sepoys on the battlefield must not be trusted with the facts and plans.
