    The top people didn't listen to the coaches who knew what the product needed to be successful.

    Realize you do not have all the answers and empower your employees to bring their ideas to fore. Do not shoot down every idea especially when you offer none of your own (CEO specific).

    flexible schedule, great relationships with coworkers, time to workout

    ongoing changes in executive leadership, especially at the CEO level
    too many changes in focus and direction
    decisions are only made by a few people
