As a Family Support Worker for Healthy Families experienced in working with children and families, you will be providing support to optimize the home environment for the physical, mental and emotional well-being of newborns up to the age of five and any siblings who are at risk of being abused and neglected. You will empower families to function successfully by assisting in strengthening the parent by traveling locally to homes at least 80% of your time.

Establish a trusting relationship with at-risk families by initiating and maintaining regular client contact through intensive home visitation and group facilitation, enhanced case management services.
Monitor quality of care by complying with contract requirements for contacting clients by phone, by letter, and/or by conducting home visits at least 80% of your time
Facilitate or co-facilitate educational classes such as parenting skills, independent living skills, anger management, household management, behavior management, pregnancy/pre-natal care etc.
Develop comprehensive Family Goal Plans for each client that clearly identifies goals and service needs.
Assist in developing and maintaining client records and program documentation according to contract and CHS standards (initial assessments, goal planning, progress notes termination summaries, etc.); enter necessary information into required software system(s) in a timely and accurate manner.
Provide appropriate modeling techniques, education and other supportive services as delineated in service plan.

Ability to have reliable transportation to conduct local home visits
Ability to commit to travel throughout Florida to attend trainings ranging from one week or less per training, mostly over the first six months
Ability to handle confidential information appropriately
Effective verbal and written communication, interpersonal skills
Ability to learn computer skills sufficient to perform all essential functions
Ability to be sensitive to the service population’s cultural and socioeconomic characteristics
Commitment to effectively and efficiently providing customer service satisfaction with positive service delivery results
Excellent parenting, organizational and listening skills
Ability to establish trusting relationships
Ability to handle emergencies and life threatening situations effectively
Ability to handle stressful situations
Ability to work flexible hours

High School diploma or GED equivalent
Associates degree preferred
Two years of experience working with children and families required
