Our team was honored to represent Chapa‑De Indian Health at the State Capitol for CPCA Day at the Capitol, an annual advocacy event hosted by CPCA Advocates and the California Primary Care Association (CPCA). CPCA represents community health centers and clinics across California that provide primary, preventive, and specialty care to millions of patients, many of whom are low‑income, uninsured, rural, or otherwise underserved. CPCA Day at the Capitol provides an opportunity for health care organizations to meet directly with state legislators and staff to share real‑world perspectives on the policies and funding decisions that impact access to care. We were grateful for the opportunity to meet with State Senator Megan Dahle and Assemblymember Heather Hadwick, as well as a staff member from Assemblymember Patterson’s office, to discuss the importance of sustaining community‑based health care and ensuring our patients can continue to receive high‑quality, compassionate services close to home. Thank you to these representatives and their teams for taking the time to listen and hear the needs of our communities. We’re proud to share Chapa‑De’s 2025 Annual Report, highlighting strong quality outcomes and an important milestone in our journey of continuous improvement. This year, Chapa‑De ranked among leading providers across multiple quality measures: 𝗧𝗼𝗽 𝟮𝟱% 𝗼𝗳 𝗣𝗿𝗼𝘃𝗶𝗱𝗲𝗿𝘀 • 71% controlling high blood pressure (ages 18 to 85) • 78% lead screening in children • 60% breast cancer screening 𝗧𝗼𝗽 𝗛𝗮𝗹𝗳 𝗼𝗳 𝗣𝗿𝗼𝘃𝗶𝗱𝗲𝗿𝘀 • 50% well child visits (ages 3 to 21) • 70% cervical cancer screening • 39% colorectal cancer screening We are also honored to be recognized for the first time in the 2024 Primary Care Provider Quality Improvement Program through Partnership HealthPlan of California. In our inaugural year, Chapa‑De placed in the top third statewide, ranking 40th out of 128 organizations, an impressive achievement for a new participant. We were grateful to welcome Cindy Rowe to Chapa‑De to speak with our managers and supervisors about leading with kindness. Her message highlighted that kindness is not just a “nice to have.” It is a real leadership skill that strengthens teams and supports better care.
