Bertrand Chaffee Hospital in Springville N.Y., a community governed non-profit organization dedicated to providing essential health resources to our community. We provide Acute, Emergency, Outpatient and Long Term Care services. Our facilities include Jennie B. Richmond Nursing Home, our Medical Arts Building in addition to the Hospital. Ancillary services include Surgical Services, on site Cardiac Care and Primary Care centers, Physical Therapy and Imaging services. Person Centered Services is a Care Coordination Organization serves over 17,000 people with Intellectual and Developmental Disabilities in 21 counties of Western New York State. Our Care Coordinators work on a "Life Plan" tailored to every one of an individual's needs. This is done by using a person-centered, holistic approach, where each person being served has their health care providers, and other service providers, collaborate to ensure their wants and needs are met. We’re in the business of helping people and always using a strengths-based approach. We work with dozens of organizations and hundreds of providers, so people can meet all of their goals. New individuals to care coordination are accepted regardless of agency affiliation, and we work with 100% of the agencies in our service area.

People’s Arc of Suffolk, a chapter of The Arc New York, has grown from a handful of volunteers into one of the region’s most trusted organizations. We are a leading advocate for people of all ages with intellectual and developmental disabilities. Our range of programs and services includes educational programs, day habilitation services, community residences, and employment opportunities both on and off-premises.
