Since the goal of Clover Meadows is to assist all patients in returning to the most independent living environment as quickly as possible, our dedicated staff helps patients reach their recovery goals successfully through an outcome-orientated approach to care. Whether recovering from surgery, coping with chronic illness, or recovering from an injury, our dedicated team of doctors, therapists, nurses and other health care professionals will work together with the patient and his or her family to design a goal-oriented plan of care for each patient. This plan will help the patient regain achieve the maximum outcome and return to the most independent living environment as quickly as possible. Clover Meadows’ sub-acute care division provides the best possible option for a quicker recovery and a return to independence. Physical therapy is offered 7 days a week, if indicated by a plan of care, to accelerate recovery times.
