The middle and lower managers are completely clueless on how to promote a healthy and efficient work environment. Management refuses to admit to their own ignorance so they compensate with arrogance. The field workers are not able to express ideas to improve safety or efficiency because they fear retaliation from immediate supervisors. Field workers are spoon fed lies and promises of a excellent safety culture with ethical leadership but are only shown the exact opposite. The only means of advancement is to stroke the egos of ones immediate superiors and not actually improve on any real issues.
